Proof of residence (one of the following)
  • Current utility bill in your name showing your residential address.
  • Recent council rates notice.
  • Rental agreement or a copy of your contract of sale when you purchased the property.
Proof of identity (one of the following)
  • Your child’s birth certificate.
  • Your child’s passport.
Proof of visa details (for non-Australian citizens)
  • Visa Entitlement Verification Online (VEVO) printout.
  • or a Department of Home Affairs visa grant letter.
Court orders
  • Copies of any court, custody or intervention orders.
